UPCOMING FLU VACCINATION CLINIC
The Adams County Health Department will be at Pikeland Community School during Parent/Teacher Conferences on October 19th from 4:00 PM to 6:00 PM to provide a flu clinic for the community. The clinic will be held in the school’s media center. Please follow the signs that will direct you to the flu clinic once entering the building.
Anyone wanting to receive the vaccine must fill out the consent form, bring their ID, and an insurance card. If insurance is through a spouse or parent, the name of the parent or spouse and their birthdate need to be put in the insurance area on the consent.
Please call the school nurse to have the form sent home with your student, or you may fill one out at the time of the clinic. Please note that the consent is 2 sided and both sides need to be filled out.
If you are going to receive a vaccine and also have a scheduled conference time, please allow enough time prior to your appointment, or choose to get your vaccine after.
